Causes: Health, Home Health Care
Mission: Homeplus care offers excellent and affordable home health care and community-based social services to individuals and families of southeastern pennsylvania. The in-home care services we provide enable our consumers to remain safely in their own homes, delay or avoid a move to institutional care , and choose socially a connected and active life. We have also developed social work service targeting immigrant caregiver and family members to support multicultural communities in our region
Programs: As a participant of medicaid hcbs programs, the hpc is now eligible to work with the comprehensive personal care services for the frail elderly (hcbs/fe), the physically disabled (hcbs-pd), developmentally disabled, and the traumatic brain injury waiver (hcbs-tbi). Our consumers are primarily persons who are at risk of hospitalization or institutionalization, such as the elderly, chronically ill and disabled. Our personal care services include assistance with bathing, grooming, dressing, personal hygiene and medication reminders, and other activities of daily living.
