Mission: The purpose of the hiv care center, inc. Is to provide the means to fill the gaps and remove barriers preventing hiv-infected individuals from staying in care and maintaining their health. Services include medication assistance, assistance with patient assistance programs for free or low cost medications, providing transportation for check-ups, counseling and/or referral for depression, drug and alcohol counseling, disease support counseling, hospitalization for un-insured patients, nutritional counseling.
Programs: The hiv care center, inc. Provides two social services staff members (1 social worker and 1 case manager) for project aids care case management, as well as linkage and support to center for prevention and treatment of infections hiv-infected patients. The center for prevention and treatment of infections saw 841 unique hiv patients, 300 of these patients were uninsured and in need of many services available only through the networked support social services provided. The medicaid, project aids care waiver program, held a monthly average of 31 patients. Criteria for this program is the diagnosis of aids by a physician, presence of aids related opportunistic infections, determined to be "at-risk" of hospitalization or institutionalization, but capable of remaining safely in the home and community. The program case management staff develop and update individualized plans of care for each patient, initial and periodic comprehensive needs assessments and identify functional and social service needs. Case management involves coordination of efforts to access services provided by all funding sources. Cm provides service authorizations to providers for services identified in the needs assessments, and follow-up with each patient to determine the quality, quantity, duration and effectiveness of service provision. Cm conducts home visits and telephone calls to the patients on a routine basis and also acts as an advocate for the needs of each patient. Linkage and education - the linkage program provides basic hiv education to 90 unique patients over the course of a year. All hiv/aids patients at the center for prevention and treatment of infections received education on preventing transmission as well as linkage to community resources including programs such as ryan white, project aids care, hopwa, adap, positive healthcare, perinatal linkage, social security, medicaid, medicare, medicare part d, pharmaceutical patient assistance programs, we care, as well as mental health and substance abuse counseling and hiv testing.
