Programs: Skilled nursing. Our skilled nursing programs are designed to help long term care residents maintain the highest possible level of function and to establish a meaningful, active life within the community of our 277-bed facility. The emphasis is always on what abilities and function remain, not on what has been lost. Vital life enrichment programs actively engage our long term residents with the community within our facility and with the outside community as well. Through a vibrant volunteer program, connections to members of the larger community, of all ages, are fostered and sustained. The presence of full-time, salaried physicians and experienced licensed and non- licensed nursing staff assures that medical and nursing needs are met and are monitored closely. Our short-term rehabilitation unit focuses on returning patients safely to their prior living settings-whether in the community, in assisted living facilities or other long term care facilities. Highly skilled rehabilitation specialists provide comprehensive short-term inpatient care for people recovering from an injury, surgery, acute illness or the exacerbation of a chronic disease. The presence of full-time geriatric aprn and physician staff enables us to monitor medical conditions and progress daily and to assure timely interventions. Skilled msw-prepared social workers work with medical and rehabilitation staff to design a safe discharge plan based on the individual's goals and capabilities. Our open visiting policy assures that both short term patients and long term residents benefit from the support of family and friends. Our skilled nursing programs are medicare and medicaid certified.
