Mission: Acting as and advocate for the individuals
Programs: Provide preventive services and healthy lifestyle education to the community to combat heart disease, diabetes and hypertension. In 2014, services and education were provided to 6894 area residents through our new beginnings education program, community access program, and our comprehensive chronic care free clinic for under-insured and uninsured. Community garden, farmer's market and farm gleanings netted over 28,000 pounds of fresh fruits and vegetables. Childhood obesity programming for families of 908 children. Senior classes for 876 participants. Free clinic outcomes continue to exceed national averages.
