Mission: To unite public and private health care providers and key community stakeholders to promote high-quality, high-value and patient-centered care for all members of the santa cruz county community with a focus on the safety net.
Programs: Strengthening the safety netsafety net clinic coalition (sncc) & sncc medical directors (sncc md)sncc and sncc md are core hip activities convened quarterly with the 2020 vision of creating a comprehensive, well-coordinated system of high quality, affordable care with an easily accessible medical home for all low-income residents. With this vision and hips focus areas in mind, we held survey discussions with both coalitions on their priorities, needs and alignment with hip efforts. 21 organizations, 75 attendees. Integrated behavioral health action coalitionin april 2015, hip launched what is now ibhac, a broad-sector community coalition which strives to create a collaborative, integrative, and inclusive system of whole person care in santa cruz county. 10 meetings, 48 organizations, 244 attendees. In recent months, this group has deepened relationships and more robustly included substance use disorders in the term behavioral health. With continued funding from blue shield of california foundation, ibhac will become a learning community to enhance understanding of addiction as a chronic condition, work toward improved care coordination, and more seamless care transitions. Technical assistance and training support for universal implementation of screening, brief intervention, and referral to treatment (sbirt) in multiple settings will improve the local standard of care. Primary care and ed clinicians will enhance their skill in motivational interviewing through the use of a self-paced, interactive, web-based training platform from kognito.
promoting collaboration:hip councilhipc is core to hips mission to provide a neutral table or platform a nexus where leaders can and do convene. Monthly meetings bring together the healthcare decision-makers in the community to inform and discuss issues that are beyond any one organization's capacity or domain, yet affect all of us. Consistent with hips direction to fully engage mental health and substance use treatment in the healthcare conversation, we are excited to welcome new hipc participant organizations encompass community services and janus of santa cruz. Kaiser permanente, also a new hipc participant, will officially commence clinical services in santa cruz county in early 2017. Complex care coordinationhips role is to lead sustainable systems change to advance care coordination for our most vulnerable residents. Hip convenes partners to tackle the challenges of complex care coordination across multiple care settings including the hospital, clinics, mental health and substance use disorder treatment and housing agencies. We are also creating a forum to test and prepare for the ca health homes program. Hip community forum the april 2017 community forum presented keynote speaker ian morrison; internationally-known futurist, consultant and author specializing in healthcare long-term planning, forecasting and its changing business environment. The event also included a celebration of salud para la gente and planned parenthood mar monte recognizing their efforts to reduce the rate of teenage pregnancy in santa cruz county.
building systems of caresafe prescribingsafe rx santa cruz county brings together patients, healthcare providers, behavioral health practitioners, prevention partners, and the community to decrease misuse and deaths related to prescription pain medications. Driven by commitments to health, safety, and compassion, representatives from fifteen agencies comprise the interdisciplinary steering committee. Continuing medical education continuing medical education events in may of 2016 hip wrapped up the alliance sponsored cme series, the changing landscape of pain management. The programs brought together experts to presented on: medication management & tapering, empathetic communication with patients, prescribing buprenorphine, neuroscience of pain & trauma informed carepractice coaching engagementship staff provide project management support for the practice coaching initiatives offered though the alliance capacity grants program. Sixteen clinic sites engaged in practice transformation focusing on aims such as: decreasing visit cycle times, redesigning referral tracking processes, improving post discharge follow up procedures, bolstering team optimization. Participants came together for a leaning collaborative event where they shared best practices and their transformation experiences with each other.
