Mission: Health Hope Network serves the communities of Western Pennsylvania through two programs - Stroke Survivor Connection and FLU B GONE. Stroke Survivor Connection is a network of free weekly therapy support groups for stroke survivors offering physical, cognitive and emotional support. FLU B GONE protects the health of Western Pennsylvania residents through influenza and pneumonia vaccination clinics every Fall.
Results: In the past 5 years Health Hope Network has administered almost 100,000 flu and pneumonia shots and has helped hundreds of stroke survivors to improve their motor and cognitive skills, overcome emotional difficulties, and live more independent lives. At any given time there are about 250 participants in the Stroke Survivor Connection program.
Target demographics: Stroke Survivor Connection: Stroke survivors in Western Pennsylvania; primarily low income and elderly.FLU B GONE: All residents of Western Pennsylvania who might benefit from an influenza or pneumonia vaccine; particularly the high-risk elderly population.
