Mission: The mission of harmony house, inc. Is to provide quality permanent and transitional housing, medical respite care and primary health care to homeless persons in texas, through a supportive, drug and alcohol free community.
Programs: The corporation provides three permanent housing programs for chronically homeless men and women (20 units, 60 units & 75 units), and a 28 unit medical respite program for homeless persons. All programs provide supportive services and case management to aid homeless individuals transitioning back into the workforce and/or independent living situations. Permanent solutions is a 75 unit permanent supportive housing program that provides case management, intensive life-skills classes and referrals to mental health counseling. Program participants are provided with resources that focus on enhancing their abilities to exit homelessness and to remain self-sufficient. Harmony house is one of the largest psh providers in houston/harris county. Harmony house collaborates with new hope housing, a low income housing provider, as well as others to provide apartments to clients within these programs. In our first year of operation we served 76 individuals, and 74 remained permanently housed. Clients with zero income have been referred to soar or va benefits specialist. Square 2 "the solution" is a 60 unit permanent supportive housing (psh)program that provides case management, intensive life-skills classes and referrals to mental health counseling. Program participants are provided with resources that focus on enhancing their abilities to exit homelessness and to remain self-sufficient. Harmony house is one of the largest psh providers in houston/harris county. Medical respite care is a 28 bed, self-contained facility that provides a clean, nurturing environment for healing. This program intervenes at the time of hospital discharge for homeless men who are too sick to be on the streets but not sick enough to be in the hospital. Patients receive intensive case management and referral to community supportive services. In-house services include a physician supervised nurse practitioner, pharmacy and substance abuse counseling provided with our collaborate partner harris health systems healthcare for the homeless program. In our most recent annual p annual progress report (apr) for 2017, we report that we served 67 individuals, of those 47 completed the program, 43 were stably housed (91%), 35 recieved income from ssi, ssdi, or earned income.
