Mission: The Foundation for Care Integration (FCI) is a nonprofit public health benefit corporation created in 2015 whose mission is to facilitate access to care to support unfunded or underfunded home and community-based services for at-risk populations. Our goal is to enable these community members to stay safely in their home or place of residence in lieu of being admitted into a health institution. In this way we are able to reduce the rate hospitalizations and/or rehospitalizations among disabled, ill, and aging adults. FCI demonstrates the effectiveness of bridging the gap between clinical healthcare and support services, creating an “integrated model” of care and access to services. Our ultimate goal is to support patients and their families across the continuum of care and to collect data that will influence reform and initiatives that embrace programs to better serve at risk populations.
Target demographics: vulnerable, fragile older adults who would not otherwise be able to afford it.
Direct beneficiaries per year: Over 160 people were assisted and hospital readmissions were less than 1%. Recipients paid only 50% of the cost for care.

FCI was there for my family in a time of need, and we are forever grateful for their services. My grandpa was on hospice and was passing. I live on the other side of the country and couldn't afford a plane ticket. They helped me pay for my flight so I could be by grandpa's side in his final moments. Thank you FCI!!!
