Programs: Since 1975, the diablo valley foundation for the aging has served over 5,000 older adults in contra costa county and has continued to be a leading agency in senior services. program service accomplishments [continued from form 990 page 2]* comprehensive needs assessment completed by a geriatric social worker* coordination and implementation of social services* monitoring and supervision of personal assistants* regular visitations and check-ins with client* advocacy on behalf of client, the family, social services, and health care providerspersonal assistants provide help with:* transportation* shopping & errands* meal preparation* personal care & hygiene* light housekeeping* companionship* medication remindersdvfa offers money management services consisting of financial planning, bill paying, reconciling bank accounts, coordinating payment of taxes, assistance in selling real estate, transfer of stocks and other investment transactions. Dvfa serves as a representative (under a durable power of attorney program) in the event of an emergency or decline in health. Executor, trustee and conservatorship services are also available. Dvfa also runs a respite center. The dvfa valley oak respite center provides an interactive program for frail seniors, those afflicted with alzheimers, related dementias and physical disabilities. Seniors have the opportunity to participate in meaningful activities, which encourage independence and fellowship.
