Causes: Job Training
Mission: To develop and provide an integrated comprehensive system of services for adults with mental retardation and a wide range of developmental disabilities. These services are designed to strengthen each individual's ability to fully functional and to improve and expand their opportunities to be independent, so that they can live with dignity, while participating in and contributing to our communities
Programs: Daybreak provided habilitative services to individuals with developmental disabilities who are in the medicaid waiver program. During fiscal 2017, daybreak provided 32,487 day habilitation services to individuals with developmental disabilities. A unit of service is measured by a set number of hours an individual spends at one of the organization's day habilitation sites working on daily activities as well as integration in the community.
daybreak provided habilitative services to individuals with developmental disabilities who live in their own home or family home. During fiscal 2017, daybreak provided 46,883 community habilitation services to individuals with developmental disabilities. A unit of service is measured by a set amount of time, and the individuals are approved for services to be rendered in their own home or in the community.
daybreak provided medicaid service coordination/case management to individuals with intellectual and developmental disabilties. During fiscal 2017 daybreak provided 579 units of medicaid service coordination. This service consists of assisting the individual and/or his/her family in coordinating and maximizing medicaid, housing, social security, and other benefits that will enhance the quality of living of the individual.
during fiscal 2016, daybreak was approved to start a residential habilitation program for individuals with developmental disabilities. No units of service were provided.
