Programs: Dakota medical foundation (dmf) and its supporting organization, dakota medical charities (dmc), (the "foundations") are recognized leaders dedicated to effectively addressing societal health problems and measurably improving the health and healthcare access for people in north dakota and western minnesota. The foundations work closely with and through hundreds of engaged partners in the region: health-related nonprofits, healthcare facilities, universities, public agencies, foundations, businesses, volunteers and donors. The foundations place particular emphasis on initiatives and projects that address the special healthcare needs of the communities it serves within its traditional service area. The foundations focused their grants and initiative programs in support of access to healthcare, chronic disease prevention, and organizational effectiveness. Access to healthcarelend a hand initiative: in 2007, dakota medical foundation established the lend a hand initiative to provide financial and fundraising assistance to a defined charitable class of families and individuals in cass county, nd and clay county, mn that experience financial hardships due to catastrophic medical issues. A 12-member application review committee determines eligibility regarding requests for up to $5,000 in foundation matching funds available to the defined charitable class of family benefit funds. In 2017, the dmf lend a hand program completed ten years of service and has assisted in raising over $13. 4 million for 435 qualified families. The lend a hand program teamed up with over 6,000 community members who coordinated fundraising benefit events in local communities with more than 150,000 people attending and supporting the events. The foundations have provided matching funds to boost benefit dollars raised totaling over $2. 5 million since 2008. Donors may contribute to: 1) the dmf lend a hand giving fund or dmf lend a hand endowment fund where contributions are matched by the foundations; or 2) directly to the community established family benefit funds that meet the program's eligibility requirements. The lend a hand program provides a step-by-step fundraising guide, a toolkit for volunteers that include planning and marketing templates, and additional resources and fundraising assistance (dmflendahand. Org). Donations to the lend a hand program may be made by cash, check or online through the impact foundation website, impactgiveback. Org. Donations to family benefit funds are not tax deductible. Children's mental health initiative: in 2007, the foundations partnered with the southeastern north dakota community action agency (sendcaa) to develop, implement and manage strategies to provide access to early social-emotional mental health assessment and therapeutic consultation services for children; improve education and awareness of early childhood mental health; and facilitate collaboration among providers. Working with local public health units, clinics and childcare sites across north dakota, but especially in cass county, nd and clay county, mn, the initiative implemented social-emotional mental health screenings, assessments and consultation services for children up to age 8. The foundations provided $25,000 in 2017 and the program dissolved in august of 2017.

chronic disease prevention the foundations joined with other like-minded organizations in 2009 to promote and facilitate a comprehensive approach for residents living in cass county, nd and clay county, mn to eat healthier and lead more active lifestyles, resulting in decreased incidence of obesity and chronic diseases. The initiative's overall strategy is to coordinate community collaboration to improve the health of citizens and to integrate best practices through the engagement of government, education, food industry, worksite wellness, community-based organizations, and insurers to make cass and clay counties the healthiest place in america to live, work, and play. Streetsalive! The healthy communities focus in 2017 included the eighth annual streetsalive! Event to encourage healthier lifestyles in the community through movement by walking, running, biking, skating, dancing and any other means of human movement. Participants used human-powered transportation to navigate the 3-mile course through the fargo/moorhead area. Over 30 community partners participated and supported the streetsalive! Effort by establishing healthy food booths, and providing health education and mini-activity classes. Two 2017 streetsalive! Events drew over 8,000 attendees. Schoolsalive! Encourages every school to design and implement a comprehensive school physical activity program (cspap) as part of the school's wellness policy. In 2017, the foundation's provided $67,090 to south east education cooperative (seec) for their healthy schools pilot that focused on implementing a cspap in 10 schools across southeast north dakotaand for the school wellness summit. An additional $12,500 was provided to elementary schools in west fargo to encourage and enhance physical movement in the classroom. Schoolsalive! Dietitian support: the foundations provided $16,300 in 2017 to support culinary trainings for school cooks that uses fresh, healthy ingredients and reduces additives and preservatives in student lunches. More than 230 individuals from 40 nd school districts and dozens of childcares have participated in culinary boot camps. The foundations also provided a $6,000 grant to the family wellness center for healthy cooking in the classroom which teaches young students how to prepare fresh, healthy meals and snacks. Childcarealive! The foundations collaborated with the childcare aware program to increase physical activity and ensure healthier eating in childcare settings. The foundations granted $75,050 in 2017 for early childhood professionals to take part in online and face-to-face training, assessment of current practices, goal setting, and actionable planning around improving children's health. Childcarealive! Has provided training to over 1,300 childcare providers and parents in functions addressing childhood obesity, increased outdoor activity, and nutritional education through 2017. Worksite wellness initiative: p5 performance is a new program which helps organizations create sustainable, supportive, and healthy cultures. By using a nontraditional evidence-based approach, p5 addresses five core constructs: purpose, people, places, practices, and performance. These constructs have shown to enhance employee well-being and organizational performance. The foundations provided $178,000 in funding for this initiative in 2017. End child sexual abuse initiative: pledge to protect is a community movement engaging every citizen in prevention. One in 10 children are currently estimated to experience sexual abuse before the age of 18. In cass and clay counties, that is 5,146 kids. And yet, incidents are grossly under-reported because sexual abuse exists as a silent and hidden epidemic. Pledge to protect will raise awareness, reduce risk, and create a safer and healthier future for our children. A total of $84,132 was provided for this initiative by the foundations in 2017.

organizational effectivenessthe foundations believe it is imperative for nonprofit organizations to build their capacity to become efficient high-impact, sustainable nonprofits to provide more effective health outcomes for the clients they serve over the long term. The foundations recognize the challenges that many nonprofit health related organizations encounter in delivering needed programs that improve health and access to healthcare. The foundations provide financial support to the impact institute training and education programs made available to nonprofit organizations. Since inception in 2005, more than 5,200 nonprofit executives and directors from over 460 nonprofits have trained in impact institute leadership and fundraising concepts. In 2017, the impact institute helped equip charitable organizations to solve complex community challenges through our training and coaching. We have trained over 300 organizations, particularly in fundraising. Many of these organizations have continued their professional development through the fundraising curriculum and a coaching program titled fundinglogic which was developed at impact institute to build a mastery of fundraising and leadership. A study of 67 nonprofit organizations that have participated in fundraising training since inception showed that their public support on giving hearts day increased over a five year period from $2. 53 million in 2013 to $5. 2 million in donations in 2017. During 2017, the foundations provided $275,000 in grants to impact foundation for training and coaching nonprofit leaders. Dmf center for excellence & innovation: in march 2013, dmf opened a newly-constructed 18,800 square foot building as a gift to the community. It serves as a nonprofit community center to educate and train area nonprofits, as well as a venue for conferences, retreats, fundraisers and gathering location for area nonprofit organizations and health leaders. The center provides high-tech conference rooms and a teaching kitchen facility for healthy food preparation demonstrations, particularly for school and child care food service staff. In 2017, 385 events were hosted by local nonprofits in the facility with over 11,000 community members in attendance. Giveback initiative: the foundations partnered with impact foundation to develop a giveback initiative that focuses on developing strategies and programs to inspire and substantially increase the giving of time, talent and treasure by donors in north dakota and western minnesota. The initiative created new ways of thinking and new approaches surrounding wealth transfer and volunteerism in north dakota and western minnesota. The giveback website (impactgiveback. Org) provides the capability for nonprofit organizations to accept online contributions from donors; share their stories; engage volunteers and record volunteer activities and coordinate event registrations. A significant commitment has been made by impact foundation to enhance the website in order to better connect donors with nonprofit organizations that fit the donor's charitable goals. The online giving website has processed over $22. 1 million for its partner nonprofits since its 2007 implementation. In 2017, the foundations provided $140,000 to impact foundation to streamline and improve this technology. Wealth transfer initiative: the foundations, partnering with impact foundation, have been a vocal force educating financial advisors and nonprofit organizations on the generational wealth transfer expected to occur between 2007 and 2061 in north dakota. According to the u. S. Wealth transfer: a golden era of philanthropy report commissioned by impact foundation and completed by the boston college center for philanthropy, an estimated $59 trillion will be transferred by 93. 6 million estates in the u. S. And approximately 220,000 north dakota households will transfer $308. 6 billion to families, heirs, charities, estate taxes, and accelerated lifetime divesture of other assets from 2007 to 2061. The initiative's purpose is to provide valuable information to nonprofit organizations so that they can develop their fundraising to individuals, who provide eighty percent of the funding to u. S. Nonprofits through private gifts. Giving hearts day initiative: the foundations partnered with impact foundation and the alex stern family foundation to establish this initiative which promotes charitable giving in the region. The online giving environment improves the capacity of area nonprofit organizations to efficiently raise funds for their programs. The online giving website at impactgiveback. Org is used to promote the annual giving hearts day each february and provides donors with an easy-to-use website to make online contributions to their favorite north dakota and minnesota charitable organizations throughout the year. Since implementation in 2008, giving hearts day has generated contributions for community nonprofit organizations totaling over $31. 3 million. The foundations have provided over $4. 3 million in matching fund grants and incentive grants for participating organizations since inception. The giving hearts day event held in february 2017 generated 63,635 donations for 360 charitable causes totaling over $10. 2 million in a 24-hour period. During 2017, the foundations provided more than $498,145 in match grants and incentives to participating nonprofit organizations. Grant writing service: the foundations support grant writing assistance for health-related nonprofit organizations to generate additional financial resources for their programs. Since the formal addition of grant writing assistance, the foundations and impact foundation have informed and advised other regional nonprofit organizations in securing over $22 million in grant funding.

innovation - the foundations committed $105,000 in funding to inspire and encourage nonprofits and people in our region to think big, sparking bold, innovative solutions to solve persistent health problems. The challenge is to identify ambitious, creative new ideas to significantly improve the health of people in our region, or to remove persistent hurdles that impact health and well-being. Innovation is the most likely path to produce breakthroughs that conquer persistent health problems. The foundations engage and challenge people and organizations to think bigger and bolder, and then supply the grant capital and guidance to explore new ideas and launch new ventures to tackle tough healthcare issues. End addiction: the mayor's blue ribbon commission on addiction is a public/private community collaborative launched by the foundations to mobilize the community around a brilliant strategy to prevent substance abuse and addiction, and to ensure access to highly effective treatment and recovery support. In 2017, the foundations provided $82,000 in funding.
