Mission: Dakota Communities partners with people who have disabilities to realize their potential in their lives and communities.
Programs: Residential services: in 2014, living well disability services provided residential services and support for 203 people with intellectual, physical and developmental disabilities living in 33 group homes in dakota, hennepin, isanti, ramsey, sherburne and wright counties. Over 600 well trained, full and part time staff provided support on a 24/7 basis. Living well disability services serves a broad spectrum of people with intellectual, developmental, and physical disabilities such as cerebral palsy, down syndrome, multiple sclerosis, autism, and brain injury. These may be complicated by health issues related to aging, diabetes, alzheimer's, and medical fragility. We also provide mental health and behavior support as part of our services for people with multiple diagnoses. Living well disability services is evolving our services as those we serve experience a variety of age-related issues including dementia and alzheimer's. Living well disability services is recognized as a high quality provider specifically for those who are medically fragile and otherwise difficult to serve in typical residential or community settings. Wellness programming contributes to success through exercise and physical fitness, nutritious menus, pet therapy, recreational therapy, and volunteer opportunities that offer meaningful community engagement.
customized services: in 2014, living well disability services provided services and support to 113 individuals and their families. Customized services includes in-home family support (ihfs), independent living services (ils) and semi-independent living services (sils) and was provided to individuals in dakota, ramsey, hennepin, washington, pine, and olmstead counties in 2014. Wellness programming is core to the services we provide and is designed to improve quality of life through physical, mental and emotional health. Wellness involves: physical; emotional; social; occupational; environmental; financial; cultural/spiritual and intellectual components.
