Programs: During the fiscal year 2016-2017curry senior center served approximately 2,500 older adults (age 55 and older) and provided a range of health, social services, nutrition and housing support. Core activities include:behavioral health programscurry senior centers behavioral health program includes both substance abuse and mental health services. Both services include individual and group counseling sessions as well as home visits to frail and homebound elders. In fy 2016-17, 53 clients received 856 hours of substance abuse services and 24 clients received 304 hours of mental health services. Case management the curry senior center case management team links elders to appropriate support, enabling them to remain living independently in their home. This includes assistance with receiving meals, homemaking, benefit paperwork, housing applications and much more. In 2016-17, 373 clients benefitted from 8,374 case management sessions. Dining roomcongregate meals are provided twice daily 365 days/year. In 2016-17, 895 seniors participated with 48,220 breakfast meals served and 70,318 lunch meals served. Total meals served was 118,538. Housing servicescurry senior center provides 13 units of permanent housing for formerly homeless seniors. During 2016-17, the overall occupancy rate was 100%. Curry also started two new housing based supportive services programs in 2016-17. Curry began providing on-site case management supportive services to 88-residents of the cadillac hotel, owned by reality house west. Curry also began a wellness nursing pilot program with tenderloin neighborhood development corporation in three of their senior buildings. Primary health care servicesprimary care services include a range of primary medical care services provided by physicians and nurse practitioners in the clinic and in clients homes, as well as podiatry, medication management and ophthalmological diagnostic testing. During 2016-17, 1,244 patients were seen and 7,101 service visits were provided. Senior centercurry senior centers community or senior center programs include a wide range of socialization, education, nutrition, translation and computer services to the tenderloin's multi-ethnic community. Programs are provided in partnership with the community living campaign, community technology network, open house, self-help for the elderly, and the shanti project. Staff also assist with translation in the primary care clinic to ensure accurate communication between medical providers and clients. During this year, 851 clients were served and 4,629 hours of service were provided. Curry also started a new outreach program for lgbt seniors in collaboration with the shanti project, serving 56 seniors with 221 service hours. Social isolation outreachcurry continued into the third year of a five-year pilot program organizing senior peers to reach out and connect with socially isolated older adults. The goals of the program is to link unconnected seniors to a variety of program services. During 2016-17, connections were made with 61 seniors with a total of 1,400 contacts.
