Mission: To enhance the quality of life for our aging community by providing services that promote well being and maintain independence.
Programs: Home delivered meals (meals on wheels) is our largest program. We prepare and deliver over 180,000 meals annually to homebound seniors across most of sonoma county. These include regular meals as well as diabetic, renal, low sodium and vegetarian selections.
our senior dining sites serve 40,000 on-site meals annualy at senior centers or senior housing sites in santa rosa, sebastopol, sonoma, windsor, rohnert park, healdsburg and cloverdale. These served meals encourage socializing among participants, as well as delivering ongoing education and essential nutrition to our active aging community.
case management services log over 5,000 service hours per year, visiting 1,000 clients in their homes to assess their individual status and needs, and ensure that community services and resources are available to help them to remain safe and independent in their own homes.
