Mission: The mission of cornerstones of care is to provide through collective strength of our agencies, an array of quality preventative treatment and support services for children and their families. Our vision is to help the children and families we serve to achieve health, happiness, confidence, competence, and self-sufficiency.
Programs: Foster care - in 2016, the foster care program provided services in cass, jackson, and johnson counties in missouri. Cornerstones improves child safety and wellness of abused and neglected children under the care of the children's division. Additionally, cornerstones finds permanent homes for these children. Services are provided to children of all ethnicities, in the parent's home, relative home, kinship home, foster home, adoptive home, residential setting, as well as group home settings. Cornerstones remedies the effects of abuse and neglect, and prevents future abuse and neglect, by partnering with the community to provide therapy, parent education, substance abuse treatment, crisis intervention, child education, and behavior intervention. Outcomes:91. 6% of children will not re-enter care32% of children will reach permanency99. 68% of children will not have a child abuse and neglect report while in custody.
nurse case management - in 2016, nurse case management coordinated health care services and provided a continuity of care for foster care children in jackson county. Nurse case management implemented in home head to toe assessments for clients and worked to continue oversight of psychotropic medications prescribed. Additionally, nurse case management leadership provided training to stakeholders and community partners with the goal of ensuring that the unique and complex healthcare needs of children in care are being met in jackson county missouri.
resource home development - our resource home development program provides foster care training and performs home studies to ensure prospective families are qualified to become licensed care providers. The program works in conjunction with the missouri children's division to find homes for children in state care. Recruitment and licensing services are provided in andrew, atchison, buchanan, caldwell, carroll, cass, chariton, clay, clinton, cooper, daviess, dekalb, gentry, grundy, harrison, holt, jackson, johnson, lafayette, linn, livingston, mercer, nodaway, pettis, platte, putnam, ray, saline, sullivan, and worth counties.
these program expenses are attributable to nondirect program expenditures for the major programs listed on part iii, line 4(a) to 4(c). In addition, the program expenses relate to the home visitation program and shared program support for affiliated agencies to further their support of the community. Revenues include reimbursements from the affiliated agencies for supporting services provided.
