Mission: Concordia community support services (ccss) was formed in the spring of 2015 to provide executive level oversight, management and strategic planning for the non-profit home and community based entities that deliver quality home health services, hospice services, and supply needs in good faith, on behalf of those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity and compassion. Ccss will perform management and policy functions and the organization is committed to providing strong leadership so that the agencies can provide the highest quality of care from nursing and therapy to specialized services such as iv therapy, wound care, palliative care, and psychiatric care, all from the comfort of the patients' home.
Programs: Concordia community support services (ccss) program service accomplishments are a testament to the mission and vision that guides the actions of those we employ, "to serve our aging community with a continuum of high quality caregiving options, provided in a christian environment, and to serve those with limited funds to the best of our ability". Ccss manages the leadership and operations of concordia visiting nurses; alliance visiting nurses, dba, vna alliance; good samaritan hospice of pittsburgh; concordia hospice of washington and concordia private care. All of these entities are 501(c)(3) non-profit entities. The care provided by the employees of these organizations goes far beyond the healthcare medical services in which we specialize. Patients and families are educated and supported through disease management as well as situations related to caring for a loved one who is aging. At ccss, our faith in god is what motivates us to serve his people. The organization is committed to mentoring and teaching; therefore, providing the highest quality of care to not only our patients but also to our employees and the members of the community in which we serve. Employees are offered training and opportunities to obtain new skills such as wound care certification, palliative care training, iv therapy certification, lymphedema therapy certification, and hospice boot camp training. Executive leadership completes an educational topic to college students at slippery rock university regarding the healthcare landscape we face in an ever-changing environment. Bible study opportunities are offered to employees and members of the community bi-annually. Weekly a tops (taking off pounds sensibly) meeting is open to employees and members of the community to offer tools and programs for healthy living and weight management. Educational radio shows broadcast 4 times a month in beaver, butler and the new castle areas. The cost of the time invested by our director of marketing is $5,795. Sample topics include: * father's day without dad, * making the transition to rehab, * what medicare does not cover, * gsh information, * complications with diabetes, * how to report symptoms to your doctor, * it's the flu season 2016, * bed sores and what are they, * important papers, * the importance of vaccines, * it's the flu season and other vaccines, * preventing and treating pneumonia, * cvn services, * after a heart attack, * heart month chf, * getting pneumonia while hospitalized, * medical equipment at home, * making medical decisions in a crisis, * preventing falls for seniors, * concordia private care, * will medicare pay for home health care, * promise you won't put me in a home. Leadership served at katie's kitchen, st. Mark's lutheran church in butler. The kitchen provides a free meal every thursday. Individuals participate in serving the meal and cleanup afterwards. Four employees of ccss (including members of the management team) participate in "servant day" with st. Luke lutheran church in cabot at a cost of $590. We gathered with a common mission "to improve the lives of others". The main event of the day was to frame two houses in the parking lot of the church which were then disassembled in sections and hauled to an underdeveloped area of the united states to be completed for a family in need. Other smaller groups sewed dresses for at-risk girls for the dress a girl around the world program, tied fleece blankets for the ronald mcdonald house in pittsburgh, pa, highlighted scriptures in bibles for inmates of a local prison, packed dry soup for local food pantries, and made christmas cards for military soldiers and families. Eight of our ccss employees and members of management serve at octoberfest and summer festival at a cost of $990. Both events are sponsored by concordia lutheran health and human care and are open to the public as a way of providing entertainment, food and refreshments and a feeling of community. One ccss employee in particular devotes an extra 40 hours per year at a cost of $800 to coordinate the kids area for each festival which includes activities, crafts and face painting, all of which are provided free to the public. Concordia community support services makes cash donations to various other 501(c)(3) organizations totaling $2,098,500. The donations are used to further the mission of caring for god's people either through charitable care that supports patient's needs or to better serve both domestic and foreign communities through christ-centered programs.
