Mission: Community Nurse Health Center is dedicated to enhancing the health of our community by serving as a community healthcare center for residents of the western suburbs of Chicago who have limited access to care. We value patient dignity, community collaboration, and prevention and wellness by delivering coordinated and integrated healthcare services and education.
Target demographics: We are here to make sure that every child and adult is able to get the medical, dental, and mental health services they need to live, thrive, and contribute to our community.
Geographic areas served: Our health center is located in downtown La Grange, IL, and easily accessible by public transportation.
Programs: Medical services: preventative and acute care and health education to 4,750 patients ages 0-85. In 2016, our professional staff provided 20,537 provider visits with our doctors and nurse practitioners. 91% of our patients live within West Suburban Cook County, and 97% of our patients are from low-income households. CNHC operates a 340B medication program which offers low cost medications to eligible patients. Other ancillary services include referral coordination for specialty and hospital care.
Dental Services: comprehensive preventative and restorative oral healthcare is provided for patients aged 0-85 in an integrated environment with our medical services. Services include oral cancer screening, periodontal treatments, tobacco use assessment, orthodontic assessment and patient education. Patient education is integrated with medical services for high risk populations such as those diagnosed with diabetes. 100% of patients are recalled for preventative care visits at 3,4, or 6 months depending on their needs.
Resale Shop: our resale shop takes donations of housewares, books, furniture, and collectibles to raise funds for our clinics. The shop is staffed in partnership with over 300 volunteers who provide approximately 15,000 hours of service.
We have been recognized as a "National Health Center Quality Leader" for the past three years by the U.S. Department of Health and Human Services, and have attained a 4-star rating from Charity Navigator for the last 4 years, for demonstrating strong financial health and a commitment to accountability and transparency.
