Mission: The mission of the community advanced practice nurses (capn) is to strengthen the lives of persons who are medically underserved and to help interrupt the cycle of poverty and homelessness for these individuals. Healthy independence is promoted through health care delivered with acceptance, compassion, and sensitivity to an individual's life circumstances. Goals:-provide opportunities for people to learn healthy adaptive life skills-create a climate of supportive health care encounters-advocate for the needs and services of the target population-promote collaborative relationships with agencies serving the needs of people-foster an environment conducive to learning for health care and social service professionals
Programs: Community advanced practice nurses, inc. Is a non-profit free clinic for individuals who have little to no access to health care in metropolitan atlanta. Capn provides free-of-charge health care services to atlanta's homeless and/or economically disadvantaged population. The clinic places special emphasis on serving women, children and youth. Capn offers a broad range of health care services including immunizations, well- and sick- care, family planning, patient education, free prescription and over the counter medications, case management, acute ambulatory care, referrals, and mental health care. Capn provides services at six (6) strategically located, shelter-based sites within metropolitan atlanta's most underserved and high risk areas. In 2017, the organization served 3,407 unduplicated individuals, provided 5,329 visits, and addressed more than 20,000 health care needs. 55% of patients were children and youth (24 and under), and 78% of adult patients were women.
