Mission: Provide services for individuals with developmental disabilities
Programs: Residential program service:ccrsi operates 4 residential facilities. We have one 16 occupant residential apartment building; two 10 occupant residential buildings; and, a 9 person group home. The residential programs provide 24 hour protective oversight (365 days/year) and training/support in activities of daily living. Staff are responsible to ensure the health and safety of the residents and support them in efforts to live in the community. Specific behavioral objectives are developed by a team of people to assist each resident in meeting individual needs and endeavors. Efforts are documented as supports happen and success measured on a monthly basis.
targeted case management (tcm) service:tcm is a service provided to all individuals found eligible for services by the missouri state department of mental health (dmh). Ccrsi served over 300 people in case management services. Most of the adult population has the service billed under medicaid following guidelines established by both the federal government and the state of missouri. About 69% of the tcm we provide is medicaid. Tcm is a service designed to assist eligible individuals find the services and supports they need and/or want. Each person has an individual plan (ip) developed by a team consisting of the case manager, the individual getting supports, family members and support providers. Success meeting goals and objectives outlined in the individual plan are evaluated monthly by the case manager and reviewed with the individual and/or family members. With the passage of the home and community based services (hcbs) settings rule through the centers for medicare and medicaid services (cms), providers that operate both hcbs programs and case management through the same organization are required to separate effective july 1, 2018 in order to provide conflict-free case management services. To ensure compliance with the hcbs regulations, central connections, inc. (cci), a new, non-for-profit organization, was formed during the year ended june 30, 2017. Cci activity remained a part of ccrsi operation until ccrsi ceased all case management services and transferred the balances related to cci effective july 1, 2017.
individual supported living (isl) service:ccrsi supports 8 individuals in isl settings. All 8 people live in the same 16 unit apartment building but each has his/her own apartment. In this congregate setting, staff provide individual supports to each person to accomplish specific goals and objectives developed for them by a team. The objectives are individualized to each person's wants/needs. We provide on-site staff - 24 hours/day, 365 days/year - to ensure the health and safety of these 8 isl's. Supports and services are documented as they happen and evaluated and measured for success monthly.
