Programs: Primary adult medical care (8,840 encounters, 4,036 patients) offers services for chronic and acute care to patients the majority of whom are medicaid-eligible, or are uninsured or underinsured. Services include comprehensive coordinated treatment of chronic diseases like hypertension, diabetes, asthma and high cholesterol plus specialty clinics in physical therapy, dermatology, neurology, ophthalmology, and rheumatology. We continue to offer pediatric services including well child visits and immunization access as well as family planning services to adolescents. The medical department received medical home recognition from the national committee for quality assurance (ncqa) in 2017. circle health services' behavioral health program provides psychiatric and psychological individual counseling services (4,446 encounters, 667 patients) to adults most of whom lack insurance for treatment of mental health problems. There are also issue-oriented counseling services available for depression. A separate substance abuse program (2,844 encounters, 243 patients) provides intensive outpatient treatment, which includes both group services and individual counseling.
