Programs: Charlotte community health clinic, inc. , cchc, a nonprofit entity providing comprehensive health care services to the low income, uninsured in mecklenburg county. The clinic began with limited clinical hours in 2001 and has grown to provide primary care, chronic disease management, womens health, health education, pediatric services, and limited mental health services. The clinic has served over 6,800 unique patients and presently serves an active patient base of approximately 4,000 individuals. Cchc employs 35 full and part time personnel and 296 volunteers to staff the clinic. In 2014, cchc enrolled 1,162 new patients and provided 20,914 patient encounters that included services for acute care, chronic disease care, preventive health, and primary care.
