Mission: To assist all older citizens within our planning and service area to maintain their freedom, independence, and free exercise of individual initiative necessary for the planning and managmenet of their own lives through the coordination of actual delivery of needed services.
Programs: Home and community based services program for medicaid eligible individuals provided so individuals could remain safely in their homes instead of entering care provider residency programs serving 138 individuals with 8,415 units of service
congregate meal service provision, home delivered meal provision, and health promotion activities were provided to underserved members of the community for the purpose of supporting independent living.
