(773) 373-1073; 2659 w. 59th street (administrative office residential services (773) 918-6200). We provide housing and rehabilitation services for adults 16 and older who have disabilities. Employment training and monitoring is also provided. Seventy-one clients are served in the integrated care facilities for developmental disabilities (icf/dd) and fifty-eight clients are served in the eight community integrated living arrangements (cilas). We serve adults with disabilities, or those who are economically disadvantaged, who are aged 16 and over. We provide vocational training and habilitation services, job placement, janitorial services, intake / outreach, vocational evaluation, developmental training, work services, work adjustment, and supportive employment services.
child welfare services: foster family care and adoption services operate out of two program sites: 7640 s. Vincennes avenue and 7718 south phillips avenue. Traditional foster care services (773) 602-2660, foster care emergency shelter services (773)356-6972. We serve wards of the state, their siblings, biological and foster parents through licensing of homes, placement, and permanency support, case management, post permanency support, and child protection. The program served 235 children this year.
educational services talent search/early intervention college preparation and placement programs, 100 east 34th street, (312)328-3543. We serve low income, first generation, 6th through 12th graders. We provide college preparation toutoring, summer school classes, cultural enrichment, act test preparation, and assistance with college admission and financial aid services. Program served 1,660 students this year. Behavioral health & clinical services is located at 2454 w. 71st street (773) 918-6100 and 2715 west 63rd street (773) 918-6100. We provide mental health crisis intervention, pre-psychiatric hospital screening assessment, in-home and officer based theerapeutic couseling, psychiatric/psychological assessments and medication monitoring for children with mental health and development disorders. We served 3,697 clients this year. Early head start / head start programs which served 826 clients providing early intervention/ child development services. Lakeside academy alternative school located at 2920 south wabash, (312)949-5010. In this program we serve high school drop outs in grades 9-12 and assist them in obtaining their high school diploma. The lakeside academy program was terminated in september 2016.
