Causes: Mental Health, Residential Mental Health Treatment
Mission: ACMH is committed to the mental and physical wellbeing of vulnerable New Yorkers.
Results: We have guided people in managing chronic mental health, substance use, and other medical conditions since 1973. Under contract with Medicaid Health Homes, the U.S. Department of Housing and Urban Development, the New York State Office of Mental Health, and the New York City Department of Health and Mental Hygiene, we provide housing, outreach, engagement and care coordination, serving over 1,500 people daily.
Target demographics: Vulnerable New Yorkers experiencing serious mental health conditions and who are formerly homeless.
