Geographic areas served: Santa Clara County

Mission:
SCFHF’s mission is to promote access to affordable, high quality health care through resource development and outreach on behalf of the underserved members of our community. SCFHF’s focus is on raising funds to support the Healthy Kids program and the Children’s Health Initiative in Santa Clara County. The Children’s Health Initiative (CHI) is a county-wide, community-based effort to ensure that all children in the county whose parents have an income at or below 300% of the federal poverty level have health coverage. The Children’s Health Initiative includes two main elements: 1) an aggressive community outreach effort to identify uninsured children and assist their parents in applying for appropriate health coverage (Medi-Cal, Healthy Families, or Healthy Kids), and 2) the Healthy Kids program, a health coverage option available for uninsured children who do not qualify for state-subsidized health coverage.

Before the start of Healthy Kids, one out of every eight children in Santa Clara County was without health coverage. Presently, there are fewer than 3% of children in Santa Clara County without regular health care support as a result of CHI and the Healthy Kids program.
