Christ Channel Network, Inc. doing business as CCN House Community Development Agency is a 501 (c) (3) nonprofit organization founded in 2002 on the belief that every homeless individual can make transformational change towards a better life. CCN House mission is to end homelessness at its roots by creating a community that inspires individual change, fosters sustainable independence, and motivates men and women to reach their greatest potential. CCN Housing program combines supportive housing with individualized relapse prevention counseling, family reunification support, vocational and educational programming, and ongoing aftercare. Over the past 8 years that CCN House has been involved in the management/development of supportive housing for the homeless, elderly and some veterans, our residents have maintained their sobriety while making remarkable personal achievements. In fact, 95% of CCN House individuals and families have successfully moved on to independent lives, have maintained their sobriety and housing stability. CCN Housing Project currently manages 80 units of supportive housing to formerly homeless men and women through ten scattered residences in the City and County of Los Angeles. Some of the individuals who come to CCN House have histories of serious substance abuse and are homeless or at risk of becoming homeless, with 90% coming directly from emergency homeless shelters, residential drug treatment programs, or off the streets. The average age of CCN House residents is 24-54 years old, 61% are men, and 68% are African-American, 23% Latino/a, and 9% Caucasian. 1 out of 4 homeless individuals in CCN Housing program is a veteran , CCN House provides supportive housing to homeless and low-income veterans in partnership with the Veterans Affairs Medical Center (VAMC) of Greater Los Angeles to assist the homeless veterans and their families, including homeless veterans returning from Iraq and Afghanistan, This proposed new project with this application will increase our housing programs for veterans in Los Angeles County for the next 15 years under the HUD-VASH program to provide 50 new units of supportive housing to veterans by 2011- 2026.

If I had to make changes to this organization, I would...
Instead of Leasing properties, I will rather purchase the properties if I have the funding, CCN House is having a 5 year lease with option to purchase on these 5th Avenue properties , These properties are vacant, and will be available for move-in by January 2nd, 2011 . The second residence will be located in Long Beach close the VA Medical Center and will provide homes for an additional 25 veterans. The residence will open its doors in February 2nd, 2011.
