Touching the lives of over 50,000 of its neighbors each year, CDM works to: feed the hungry by providing over 1 million pounds of food annually through a network of food pantries and youth programs; healing the sick by distributing over 20,000 prescriptions per year to uninsured patients; housing the homeless by providing case management and housing to hundreds of youth who are transitioning out of foster care as well as the chronically homeless; and renewing hope in the lives of those struggling with poverty by offering legal services, educational training, workforce development and other programs to those in need.

Transition resource action center (trac) provides a "one stop" assessment/intake and referral facility for transitional living services for youth exiting foster and juvenile care in north texas. Trac brings together community resources and intensive case management service. It is the only such program in north texas that exists to comprehensively address the needs of this population. Trac is the regional safety net for former foster youth once they are on their own. Annually, trac helps over 800 of the most challenged at-risk teens make a life plan and acquire skills they need to survive on their own after they turn age 18.
food service programs - citysquare's summer food service ("sfsp") and child and adult care food program ("cacfp") provide meals year-round to children and youth living in poverty. In 2014, the programs operated year-round in areas surrounding three major texas cities - dallas, austin and waco - and also in houston, texas and denver, colorado over the summer months. community health services (chs) partners with families and individuals to create clear movement toward better health and quality of life. Chs provides high-quality primary, preventive pediatric, chronic and non-emergency acute care to approximately 2,000 uninsured neighbors.
