Improving the health of people of western new york.


The p2 collaborative of western new york (p2) knows that there are many things that affect the health of western new yorkers. P2 creates the healthiest community through quality improvement initiatives, community health planning, and health engagement programs. The work of p2, together with their multi-stake holder partnerships, is focused in the following four areas, taking a comprehensive approach to improving health. - working with communities to determine how to make their neighborhoods healthy places to live and work- improving services to underserved patient populations - helping physicians, practices, and hospitals to improve the quality of care they deliver

- coordinating programming to inspire western new yorkers to live healthier every day. The p2 core values include:1) equity - every western new yorker deserves to be healthy2) collaboration which drives faster, more sustainable progress3) innovation that enables true transformationp2 has the following three core disciplines:* care transformation - improve the quality of care - improve access to care - focus on underserved populations

* community health improvement - help communities to understand areas where they need improvement - provide facilitation services, data support and assistance with resources - promote policy change* health engagement - coordinate programs that empower patients to become active partners in their care, and take responsibility for and act on their own wellness
