The Children’s Health Fund (CHF) is committed to providinghealth care to the nation’s most medically underserved children and theirfamilies through the development and support of innovative primary care medicalprograms, response to public health crises, and the promotion of guaranteedaccess to appropriate health care for all children. Through our unique mobile medical units andfixed-site clinics, we offer a “medical home” to thousands of homeless and low-incomechildren and their families across the United States each year.
Children’s Health Fund (CHF) develops and supports a National Network of 22 programs and two affiliates in 15 different states and the District of Columbia. Seventeen of the programs/affiliates are based at academic medical centers, the other seven at Federally Qualified Health Centers.
Programs provide comprehensive health care through an enhanced medical home model to medically underserved children. This model integrates intensive primary care, mental health and oral health services through innovative practices, such as the use of mobile clinics, school-based clinics and telehealth.
Affiliates incorporate one of CHF’s special initiatives into their practice in order to fulfill the goal of providing an enhanced medical home. Special Initiatives include: the Referral Management Initiative (RMI), the Childhood Asthma Initiative, the Transportation Initiative, Medical Home Initiative and Starting Right (Obesity Prevention).
