Center for Elders' Independence, Inc. Overview
Target demographics: We help frail seniors in the East Bay (Hayward, Oakland, Richmond and the surrounding areas) who would otherwise be in nursing homes.
Direct beneficiaries per year: ~500 frail seniors
Geographic areas served: West Contra Costa County, Northern and Central Alameda County (San Francisco Bay Area, California)
Programs: Center for Elders' Independence program participants receive the following medical and non-medical services. Medical services include but are not limited to: Physician, specialist and nursing care, prescription medications, professional services such as dentistry, optometry, podiatry, audiology, psychiatry, laboratory and other diagnostic services.
Hospital care, emergency and nursing home care, durable equipment such as walkers, hospital beds and wheelchairs, and rehabilitative therapies are all included.
Non-medical services include but are not limited to: activities-recreational therapy programs including art, drama, celebrations, music, exercise, support groups and field trips. Home care assistance with personal care, nutritional counseling, social services counseling, assistance with benefits and family support are included. Transportation to and from the PACE Centers, medical appointments and field trips is also provided.
We serve nearly 500 seniors at four locations in Oakland and Berkeley.
CEI is a provider of the Program of All-inclusive Care for the Elderly (PACE) and is licensed by the California Department of Health Services as a primary health care clinic, adult day health center and home care agency.
