Mission: The mission of axis community health (axis) is to provide quality, affordable, accessible, and compassionate health care services that promote the well being of all members of the community. Axis' services include pediatrics, adult internal medicine, women's health care, mental health counseling, drug and alcohol services, wic nutrition services, and community health education.
Programs: Primary medical care: axis community health's (axis) primary care services consist of a medical team of licensed, board-certified family practitioners, internists, pediatricians and nurse practitioners. Axis provides comprehensive primary medical care from newborns to seniors. Axis provided over 40,000 visits to under or non-insured patients in the tri-valley community, including high-quality preventative conditions such as diabetes, asthma or high blood pressure.
drug and alcohol counseling: axis community health drug and alcohol services consist of a staff of certified drug and alcohol counselors offering a variety of addiction counseling services including: drug and alcohol recovery for adults and teens, drug and alcohol testing services, intensive outpatient treatment programs, and court ordered education programs. Axis provided over 25,500 group and individual counseling visits for adults and teens in the tri-valley community on a voluntary or court ordered basis under both public and privately funded circumstances. Axis is accredited by carf (commission on accreditation of rehabilitation facilities) as a provider of outpatient drug and alcohol treatment for adults and teens.
mental health counseling: axis community health mental health services consist of a certified, licensed staff including and medical director, psychologist, mft, lcsw, master's and phd level interns, caade certified persons, and registered addition specialists providing group and individual counseling and education assistance. Axis provided over 7,000 group and individual counseling visits for mental health issues that include coping with depression, promoting a healthy lifestyle, rebuilding family communication and cohesiveness, parenting education and strategies, anger management and crisis resolution, and domestic violence control strategies.
women, infant, children (wic: the women, infants and children supplemental nutrition program (wic)) helps eligible families improve their health status through better nutrition, supplemental food and referrals to health care services.
